      I am supporting an initiative that seeks to improve how we measure important aspects of people’s health and wellbeing.

      The Equitable People-Centred Health Measurement (EPHM) project (healthyqol.com) is developing a cutting-edge online tool for health surveys that are short and tailored to each person’s unique experiences and circumstances. To do so, we need to collect information from many different people about their circumstances, healthcare, health and wellbeing.

      I am inviting you to participate in this project. This would involve completing several surveys over a period of 6 months. If you participate, you will be eligible to receive a $10 e-gift card after completing the first set of questionnaires and another $10 e-gift card after completing the last questionnaire. Your participation is voluntary.

      To participate, you will need to:
      • Provide your informed consent via the above link.
      • Setup an online account with Cambian. This will only take a minute. Cambian is the Canadian online health information provider for this project.
      • Complete the surveys online (using your computer, phone, or tablet) or by phone (using our toll-free number).
