I think this Canadian study is interesting, and I wonder about the data that patients supply to their doctors through the program. It sounds like classic patient supplied data, that enters the health record.

The tool was co-designed. Next steps by researchers include refining the ePRO tool to better support self-management efforts in primary care.

mHealth technology can enable the integration of self-management support into an individual's daily routine by allowing them to access educational materials, record health behaviors, track health data (blood glucose or blood pressure readings) on an ongoing basis and share that information with their primary care providers to allow for collaborative management of their chronic conditions.
