          Hi Everyone,

          When I originally posted the information requested by the University of Western Ontario's Schulich School of Medicine and Dentistry regarding the designing of the online training program for POR, I do not believe I pasted the website location for your input as I thought it was in the body of the letter from Western.

          Here is a link to a short five question survey that will take approximately 5 minutes to complete. We would be grateful for your participation. To date many researchers but few patients (2) have responded.

            It was an interesting survey, and did not take long. The information letter is also included on the survey website.

              i like the conversation guide developed by Type 1 Diabetic Think Tank which applies in many areas of health care. These are the kind of questions the patient would like have HCP/researchers ask. It gives the patient the control.

              CLINICAL CONVERSATIONS THAT TRULY MATTER

              1. What is the most important issue that you would like to discuss?

              2. What is something that I find truly challenging to manage?

              3. Do I have goals and what is challenging for me to reach these goals?

              4. What do I feel that I am doing well?

              5. How can my health team best help me?

              6. Here are other items I would like to discuss.

                I was able to save it as a picture by right clicking on it. I'll try to put it in the community filing cabinet.

                Couldn't do that, so I put the picture into the Photo Gallery on our groupsite. Success!

                  Hi Annette and PAN membership.

                  I am sending along the results of your effort to have our membership complete the questionnaire from the University of Western Ontario's Primary Healthcare Researchers that will help to shape the online POR training. It is great to see more patients engaged in helping to shape meaningful patient engagement in research and researchers seeking to better understand the means and methods of engagement that improves patient outcomes. The principal researchers termed the response "amazing".
